The following forms need to be completed in order to begin enrollment of your child(ren): 

• Health Form
• Medical Consent
• Child Information Form
• Payment Policy Agreement
• Original Birth Certificate

You must also submit a one-week security deposit prior your child(ren) start date. No credit check is given.
